Introduction to Metronidazole
I. Basic information

II. Physical and chemical properties
Appearance: White to slightly yellow crystals or crystalline powder, with a slight odor, bitter and slightly salty taste.
Solubility: Slightly soluble in water, easily soluble in ethanol and chloroform, slightly soluble in ether.
Stability: Stable in a dry environment, but easily decomposed under strong acid or alkali conditions, should be kept away from light and sealed.
III. Pharmacological action and mechanism
Antibacterial spectrum:
Anaerobic bacteria: It has a strong antibacterial effect on Bacteroides fragilis, Streptococcus peptococcus, Clostridium perfringens, etc.
Protozoa: It has a killing effect on Trichomonas vaginalis, Amoeba, Giardia lamblia, etc.
Mechanism of action: Nitro groups are reduced to cytotoxic metabolites, which inhibit pathogen DNA synthesis and lead to its death.
IV. Clinical application
Infectious diseases:
Anaerobic infections: such as abdominal infections, pelvic infections, lung abscesses, brain abscesses, etc.
Trichomonas vaginitis: The first choice of drug, which should be treated by both husband and wife.
Amebiasis: Treatment of intestinal and extraintestinal amebiasis (such as amoebic liver abscesses).
Giardiasis: The first-line drug for intestinal parasitic infections.
Other uses:
Combined with antibiotics for eradication of Helicobacter pylori (Hp) infection (such as triple therapy or quadruple therapy).
Topical preparations (such as gels and creams) are used to treat rosacea (rosacea) and inflammatory acne.
V. Dosage forms and specifications
Oral preparations: tablets (0.1g, 0.2g, 0.4g), capsules (0.2g), sustained-release tablets.
Injection: 0.5g/100ml (intravenous drip).
Topical preparations: gel (1%), cream (0.75%~1%), vaginal suppository (0.5g).
Others: gargle, lotion, etc.
VI. Dosage and administration (regular adult dose, to be adjusted according to doctor's advice)
Anaerobic infection:
Oral: 0.4~0.6g/time, 3 times/day, course of treatment 7~10 days.
Intravenous drip: first dose 15mg/kg, maintenance dose 7.5mg/kg, once every 6~8 hours.
Trichomoniasis:
Oral: 0.2g/time, 3 times/day, for 7 consecutive days; or a single 2g dose (couples need to be treated together).
Amebiasis:
Intestinal amebiasis: 0.4~0.8g/time, 3 times/day, course of treatment 5~10 days.
Amebic liver abscess: 0.4~0.8g/time, 3 times/day, treatment course 3~4 weeks.
